Schedule:

• Flexible shifts available seven days a week

• Meet a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual health risk assessments in patients homes, with each evaluation averaging about 45 minutes
• Review and assess prior and current medical history, conditions, and medication use
• Deliver clear patient education regarding medical diagnoses and medication regimens
• Identify gaps in care and recommend appropriate solutions
• Note: No treatment planning or medication prescribing is required for this position

Qualifications:

• Active Nurse Practitioner license in West Virginia
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• A minimum of 6 months recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
