Job Description

Schedule and Flexibility

  • Create your own shift schedule, choosing from Monday through Sunday
  • Flexible hours to fit your availability
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities

  • Conduct thorough annual home health risk assessments during home visits
  • Evaluate medical histories, current health status, conditions, and medication profiles
  • Educate patients on medical conditions and medication management
  • Identify care gaps and offer recommendations to improve overall health outcomes
  • Complete each assessment in approximately 45 minutes
  • No treatment plan development or prescribing required

Qualifications

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing program
  • At least 6 months of current NP experience preferred

This role offers compensation of $80-$90 per evaluation, along with a mileage stipend and pay for administrative time. Your home visits will be scheduled throughout Wilkes County, including North Wilkesboro, Wilkesboro, and Ronda, allowing you to make a difference to a broad local population.
