Empower patients in Ouachita County by providing crucial healthcare assessments as a Nurse Practitioner specializing in home health. - Choose weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members, both for annual and post-discharge visits
- Evaluate patients prior and current medical history, ongoing conditions, and medication use
- Provide patient education regarding conditions and their medications
- Identify gaps in care and recommend solutions to support healthier outcomes
- No responsibility for creating treatment plans or writing prescriptions
Qualifications:
- Active Nurse Practitioner license in Arkansas (AR)
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of nurse practitioner experience preferred
This position offers compensation of $80$90 for each risk assessment completed, along with a stipend for mileage and administrative time.
