Job Description

Advance your career as a Nurse Practitioner specializing in home health by conducting risk assessments throughout Lake County, IN. This per diem opportunity offers the flexibility to create your own schedule, with available shifts between Monday and Sunday. Assignments may include locations in North Township and the communities of Gary, Hammond, and Merrillville. Each visit typically lasts approximately 45 minutes and may involve annual health evaluations or post-discharge assessments for Medicare Advantage plan members.

Key Qualifications:

  • Active Nurse Practitioner License in Indiana
  • Minimum six months of current experience as a Nurse Practitioner preferred
  • Board Certification (AANP or ANCC)
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current CPR certification (BLS)

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create and manage your own schedule
  • Minimum commitment required weekly or bi-weekly

Primary Responsibilities:

  • Conduct comprehensive annual or post-discharge home health risk assessments on Medicare Advantage members
  • Review and evaluate prior and current medical history, medical conditions, and medication lists
  • Provide patient education regarding chronic conditions and medications
  • Identify care gaps and recommend solutions to improve patient outcomes
  • No prescribing medication or developing treatment plans required

Compensation includes $80$90 per completed evaluation, with an additional mileage stipend and payment for administrative time.
