Job Description

Seeking a skilled Nurse Practitioner with experience in home health to perform thorough risk assessments for Medicare Advantage members throughout Lake County, Indiana. Schedule:

  • Flexible shifts available Monday through Sunday

Key Responsibilities:

  • Perform comprehensive home health risk assessments, generally lasting about 45 minutes, in patients homes
  • Complete assessments for various visit types including annual health evaluations and post-discharge check-ups
  • Review patients medical histories, current health conditions, and medication usage
  • Educate patients about their medical conditions and medication management
  • Pinpoint gaps in care and make clear recommendations to enhance healthcare delivery
  • No requirement to prescribe medication or formulate treatment plans

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current certification in CPR (BLS)
  • Board certified through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of Nurse Practitioner experience is preferred

This role offers strong compensationeach evaluation pays between $80 and $90, and includes a mileage stipend and payment for administrative time.
