Job Description

Seeking an experienced, patient-focused professional to deliver home health risk assessments for Medicare Advantage plan members throughout Elmore County. This role is ideal for a knowledgeable Nurse Practitioner passionate about providing quality care directly in patients homes, allowing for flexibility and autonomy in your workday.

Schedule:

  • Flexible shifts available seven days a week
  • Self-schedulingdesign your own work calendar
  • Minimum weekly or bi-weekly hourly commitments

Responsibilities include:

  • Performing comprehensive annual risk assessments, evaluating both medical history and current health status for homebound members
  • Assessing medical conditions, reviewing current and prior treatment records, and thoroughly documenting findings
  • Educating members about their medical conditions, medications, and care plans
  • Identifying care gaps and recommending appropriate follow-up or specialist resources as needed
  • Visits will not require treatment planning or prescription authority

Desired qualifications:

  • Active Nurse Practitioner Licensure in Idaho
  • Board certified via AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited institution
  • At least 6 months of current experience as a Nurse Practitioner is preferred

Compensation for this per diem role includes evaluation-based pay of $80-$90 for each completed assessment, with an included mileage stipend and administrative time allowance.
