Make a meaningful impact by providing in-home healthcare services to Medicare Advantage plan members throughout Scotland County. This per diem position offers nurse practitioners the autonomy to conduct comprehensive home health risk assessments while enjoying a high degree of scheduling flexibility.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct thorough annual risk assessments in patients homes, primarily in Scotland County neighborhoods such as Laurinburg, Wagram, and Gibson
- Review and evaluate each members health history, existing conditions, and medication regimen
- Deliver personalized education about medical conditions and medication management
- Identify any gaps in care, offering recommendations to help optimize patient outcomes
- No requirement to develop treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- Six months or more current experience as an NP preferred
- MSN from an accredited school of nursing
This role provides compensation of $80$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
