Advance your practice as a Nurse Practitioner serving Medicare Advantage members through home health risk assessments in New Haven County, CT. This per diem opportunity offers flexible scheduling, allowing you to set your own hours and manage your availability between Monday and Sunday, with a minimum weekly or bi-weekly time commitment.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingcreate your own work schedule
- Minimum commitment of weekly or bi-weekly hours
Key Responsibilities:
- Complete comprehensive home health risk assessments, each lasting approximately 45 minutes
- Review and evaluate prior and current medical history, existing conditions, and medication usage
- Deliver patient education regarding medical conditions and medication management
- Identify care gaps and provide tailored recommendations for follow-up
- Assignments may include annual assessments or post-discharge visits; no prescription or treatment planning required
Qualifications:
- Active Nurse Practitioner license in Connecticut
- Board certification from AANP or ANCC
- Current BLS certification
- MSN from an accredited nursing program
- Six months or more of current experience as a Nurse Practitioner is preferred
