Job Description
Deliver high-impact care while enjoying flexible work arrangements as an experienced Nurse Practitioner specializing in home health assessments. This per diem opportunity allows professionals to use their advanced clinical expertise to support Medicare Advantage members throughout Fairfield County, including Bridgeport, Stamford, and Norwalk.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-schedulingdesign your own work calendar
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients medical histories, current health status, and medication regimens
- Educate patients about their conditions and prescribed treatments
- Identify care gaps and recommend steps for improved health outcomes
- Focus exclusively on assessmentsno need to create treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in Connecticut
- MSN degree from an accredited nursing program
- Board certification via AANP or ANCC
- Current CPR (BLS) certification
- At least six months of nurse practitioner experience preferred
This role offers the chance to work independently while impacting patients well-being in the comfort of their homes. Nurse practitioners will be compensated $80$90 per evaluation, in addition to a mileage stipend and payment for administrative time.
