As a Nurse Practitioner specializing in Home Health, you'll visit patients throughout Erie County, conducting assessments that help identify and address care needs, prevent hospital readmissions, and support overall wellness.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

• Perform comprehensive annual health risk assessments during home visits
• Evaluate past and current medical history, physical conditions, and medication regimens
• Educate patients on their medical conditions and medications, empowering them to manage their health effectively
• Identify gaps in care and make appropriate recommendations to bridge these gaps
• No need to prescribe medication or develop ongoing treatment plansfocus is on assessment and education

Desired Qualifications:

• Current Nurse Practitioner license in New York
• Active Board Certification through AANP or ANCC
• CPR certification (BLS)
• Minimum of 6 months current experience as a Nurse Practitioner is preferred
• Master of Science in Nursing (MSN) from an accredited institution
