Bring your expertise as a Nurse Practitioner to a dynamic per diem opportunity focused on home health risk assessments for Medicare Advantage plan members across Boone County, MO. This position centers on conducting thorough home-based evaluations within Columbia, as well as the surrounding communities of Rocheport, McBaine, and Ashland. Each assessment typically lasts about 45 minutes and may address annual health reviews or post-discharge needs, depending on the patient visit.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule for weekly or bi-weekly commitments
Key Responsibilities:
- Perform comprehensive annual home health risk assessments
- Review and document detailed medical history, current health conditions, and medication regimens
- Deliver education to members regarding medical diagnoses and medication management
- Identify any healthcare gaps and propose evidence-based recommendations
- Note: No prescribing or development of treatment plans required
Qualifications:
- Current, active Nurse Practitioner license in Missouri
- Board certification through AANP or ANCC
- MSN from an accredited nursing program
- Current BLS certification
- At least 6 months of recent NP experience preferred
In this role, you will receive $80-$90 per evaluation, plus a mileage stipend and compensation for administrative tasks. This position offers you the autonomy to manage your own schedule and support the health of diverse members in the Boone County area.
