Make a meaningful impact delivering advanced care to patients right where they live. A per diem Nurse Practitioner opportunity is open for professionals passionate about personalized home health. Youll conduct in-home risk assessments for Medicare Advantage plan members across Halifax County, providing valuable insights to help improve care outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling with the ability to design your workweek
- Minimum weekly or bi-weekly hourly commitments
Responsibilities:
- Complete comprehensive home health risk assessments, typically lasting about 45 minutes each
- Review prior and current medical history, ongoing conditions, and medication lists
- Educate patients on health issues and prescribed therapies
- Identify care gaps and share recommendations for addressing them
- Visits may include annual health evaluations or post-discharge assessments
- No requirement for creating treatment plans or prescribing medication
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification (AANP or ANCC)
- Current CPR (BLS) certification
- Master of Science in Nursing from an accredited program
- At least six months of recent Nurse Practitioner experience preferred
Expect compensation of $80$90 per evaluation along with a mileage stipend and pay for required administrative time.
