Make a direct impact on patient well-being by providing comprehensive home health risk assessments in Clark County, AR. This per diem position offers significant flexibility, with shifts available seven days a week and the opportunity to build your own schedule for optimal work-life balance. As a Nurse Practitioner conducting home visits for Medicare Advantage members in locations such as Arkadelphia, Gurdon, and Amity, you will play a crucial role in assessing health conditions, delivering patient education, and recommending necessary follow-up referrals or care measures.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate medical history, current healthcare conditions, and medication regimens
- Deliver patient education regarding chronic conditions, disease management, and prescribed medications
- Identify gaps in care and provide referrals or recommendations to address health needs
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR/BLS certification
- Minimum of 6 months experience as a Nurse Practitioner (preferred)
