Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete comprehensive annual home health risk assessments for Medicare Advantage plan members in locations throughout Albany County
• Review patient medical histories and current health conditions during approximately 45-minute visits
• Provide patient education regarding chronic conditions and medications
• Assess for potential gaps in care and make recommendations, supporting continuity of care
• Visits may varyfrom annual health assessments to post-discharge follow-ups
• No responsibility for treatment planning or medication prescribing

Qualifications:

• Active Nurse Practitioner license in New York State
• Masters degree in Nursing (MSN) from an accredited institution
• Current Board Certification through the AANP or ANCC
• Current CPR certification (BLS)
• At least 6 months of recent nurse practitioner clinical experience preferred
