Advance your nurse practitioner expertise in a flexible, patient-focused setting. This per diem opportunity involves conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Albany County, including areas such as Cohoes, Watervliet, and Westmere. Each assessment is typically 45 minutes long and may include annual health evaluations or post-discharge visits, providing an opportunity to make a significant impact on patient wellness in the home environment.
Schedule:
- Flexible shifts offered between Monday and Sunday
- Create your own schedule to suit your availability
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical histories, conditions, and medication use
- Provide clear, patient-centered education related to medical conditions and medications
- Identify care gaps and recommend strategies for improved health outcomes
- No prescribing or creation of treatment plans involved
Qualifications:
- Active Nurse Practitioner license in New York State
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR (BLS) certification
- At least 6 months of NP experience, preferably in home health (preferred but not required)
Those performing evaluations will receive $80$90 per visit, along with a stipend for mileage and administrative time.
