This position involves traveling throughout the greater Franklin County areaincluding communities such as Columbus, Dublin, and Grove Cityto deliver care directly in patients homes.

Schedule:

• Flexible shifts, Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members
• Review patient medical histories, ongoing medical conditions, and current medication regimens
• Educate patients on their medical conditions and medications
• Identify gaps in care and offer actionable recommendations
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner (NP) license in Ohio (OH)
• Board Certification (AANP or ANCC)
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• Minimum six months of recent Nurse Practitioner experience preferred
