This per diem opportunity focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members across Limestone County, which encompasses Groesbeck, Mexia, and Coolidge. Youll enjoy a high level of autonomy while supporting patients through annual health evaluations and post-discharge follow-ups.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to design your own schedule for a healthy work-life balance
- Meet minimum weekly or bi-weekly hourly commitments in a way that suits you
Key Responsibilities:
- Perform thorough home health risk assessments, typically lasting about 45 minutes per visit
- Evaluate patients' current and past medical history, medical conditions, and medication usage
- Offer clear education to patients regarding their medical conditions and medication regimens
- Identify potential care gaps, providing actionable recommendations to improve patient wellbeing
- No medication prescribing or treatment plan development required
Desired Qualifications:
- Active Nurse Practitioner license in Texas
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- At least 6 months of recent NP experience preferred
- MSN degree from an accredited program
