Make a meaningful impact in patient care by delivering home health risk assessments for Medicare Advantage members throughout Gadsden County, FL. As a Nurse Practitioner in a per diem, home health-focused role, youll enjoy autonomy, flexibility, and the chance to improve health outcomes for members in communities including Quincy, Chattahoochee, and Havana.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling based on your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual health risk assessments for Medicare Advantage plan members in their homes
• Review prior and current medical history, chronic conditions, and medication regimens
• Provide essential education on medical issues and medication best practices
• Recognize care gaps and share actionable recommendations to resolve them
• Visits will not require crafting treatment plans or prescribing medicationfocus is on evaluation and education
• Each home visit typically lasts around 45 minutes

Qualifications:

• Active Nurse Practitioner license in the state of FL
• Board Certification via AANP or ANCC
• Current BLS/CPR certification
• MSN from an accredited nursing program
• At least 6 months of recent experience as an NP is preferred

In this role, you will receive competitive compensation of $80-$90 per completed evaluation, along with a mileage stipend and pay for administrative time.
