Job Description
Make a meaningful impact in patient care by delivering home health risk assessments for Medicare Advantage members throughout Gadsden County, FL. As a Nurse Practitioner in a per diem, home health-focused role, youll enjoy autonomy, flexibility, and the chance to improve health outcomes for members in communities including Quincy, Chattahoochee, and Havana.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling based on your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual health risk assessments for Medicare Advantage plan members in their homes
- Review prior and current medical history, chronic conditions, and medication regimens
- Provide essential education on medical issues and medication best practices
- Recognize care gaps and share actionable recommendations to resolve them
- Visits will not require crafting treatment plans or prescribing medicationfocus is on evaluation and education
- Each home visit typically lasts around 45 minutes
Qualifications:
- Active Nurse Practitioner license in the state of FL
- Board Certification via AANP or ANCC
- Current BLS/CPR certification
- MSN from an accredited nursing program
- At least 6 months of recent experience as an NP is preferred
In this role, you will receive competitive compensation of $80-$90 per completed evaluation, along with a mileage stipend and pay for administrative time.
