Are you a dedicated Nurse Practitioner with a passion for home health care? This per diem opportunity offers the chance to deliver patient-centered care by conducting home health risk assessments for Medicare Advantage plan members across Multnomah County, including Gresham, Troutdale, and Fairview.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual health and post-discharge risk assessments in patients homes
- Review and evaluate medical history, current conditions, and medication regimens
- Provide patient education regarding health conditions, medications, and preventive measures
- Identify gaps in care and make recommendations to enhance patient outcomes
- No treatment plan formulation or prescribing requiredfocus is on assessment and education
Qualifications:
- Active Nurse Practitioner license in Oregon
- Board Certified through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing institution
- At least 6 months of recent experience as an NP preferred
