Experience a rewarding opportunity as a Nurse Practitioner specializing in home health risk assessments in Clackamas County, OR. This per diem role offers the autonomy to create your own schedule and deliver high-quality care to Medicare Advantage plan members in locations such as Lake Oswego, Oregon City, Tualatin, and West Linn. With each visit averaging about 45 minutes, your expertise will make a significant impact on patients lives while accommodating your preferred work-life balance.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Thoroughly review patient medical histories, current conditions, and medication use
- Educate patients on medical conditions and appropriate medication management
- Identify gaps in care and recommend actionable solutions
- Visits may include annual health assessments or post-discharge follow-up
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Oregon
- Board Certification (AANP or ANCC)
- MSN from an accredited nursing program
- Current CPR/BLS certification
- At least six months of nurse practitioner experience preferred
As part of this role, you will receive $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative time. Deliver quality patient care without extensive administrative burden or after-hours demands.
