Job Description

Deliver comprehensive healthcare services as a Nurse Practitioner focused on providing quality care to senior living and homebound insurance members in Gresham, OR. This contract role is centered on monthly (or more frequent) health assessments aimed at preventing unnecessary hospitalizations and emergency room visits by treating patients in place whenever possible.

Key Responsibilities:

  • Conduct thorough health assessments and develop individualized problem lists and treatment plans
  • Provide primary, preventive, urgent, and acute care in patients homes or senior living communities
  • Identify and stratify high-risk patients to enable more frequent clinical visits and tailored care
  • Establish contingency plans to manage high-risk conditions effectively
  • Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing (DON)
  • Support care transitions by performing single home visits following discharge from skilled nursing facilities (SNF)
  • Deliver hands-on bedside care and create care orders aligned with patient needs

Qualifications:

  • Master of Science in Nursing (MSN) degree
  • Valid Oregon Nurse Practitioner (NP) license and DEA certification
  • Basic Life Support (BLS) certification
  • Experience in home health, hospice, or related settings preferred

Schedule:

  • Monday through Friday, 8 AM to 5 PM
  • No weekends or holidays, allowing work-life balance
