Nurse Practitioners with home health experience have an excellent opportunity to deliver meaningful care through in-home risk assessments for Medicare Advantage plan members. This per diem role offers flexibility, autonomy, and focuses on serving individuals across Hendricks County, INincluding Plainfield, Brownsburg, and Avon.
Schedule:
- Flexible shifts between Monday and Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct home health risk assessments (approximately 45 minutes each) for Medicare Advantage members
- Assess prior/current medical history, chronic conditions, and medication regimens
- Deliver patient education on health conditions and medications
- Identify gaps in care and make focused recommendations
- Visits may include annual health assessments or occasional post-discharge follow-up
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current CPR (BLS) certification
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing school
- At least 6 months recent NP experience preferred
This position offers compensation of $80-$90 per completed evaluation, plus mileage reimbursement and an administrative time stipend.
