Job Description

This position offers a unique opportunity for an experienced Nurse Practitioner to deliver patient-centered care through home health risk assessments for Medicare Advantage plan members in Crawford County, AR. Per diem scheduling and a flexible structure allow you to craft shifts that match your availability and lifestyle. Youll interact with diverse members of the local community, supporting comprehensive annual and post-discharge visits, each averaging 45 minutes.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Self-scheduling for maximum flexibility
  • Minimum commitment requirement on a weekly or bi-weekly basis

Responsibilities:

  • Conduct thorough home health risk assessments, focusing on annual visits and post-discharge evaluations
  • Review and evaluate patients medical histories, including chronic and acute conditions, as well as their current medication regimen
  • Provide in-home health education, empowering patients to manage their wellness and medications safely
  • Identify and document any gaps in care, offering actionable recommendations for follow-up
  • Please note: No prescribing or treatment plan development required

Qualifications:

  • Active Nurse Practitioner license in the state of Arkansas
  • Board certification (AANP or ANCC)
  • Current CPR/BLS certification
  • MSN degree from an accredited institution
  • At least six months of recent nurse practitioner experience preferred

This role comes with competitive compensation: earn $80$90 per evaluation, and receive a mileage stipend along with compensation for required administrative time.
