Job Description

Advance your Nurse Practitioner career with a dynamic per diem opportunity conducting home health risk assessments in Crawford County, AR. In this vital role, you'll work directly with Medicare Advantage plan members in locations such as Van Buren, Alma, Mulberry, and neighboring areas. Each comprehensive, in-home evaluation typically takes about 45 minutes and may include annual health assessments or post-discharge visits. Your clinical expertise will help ensure seamless, thorough care coordination and improved patient outcomes.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own schedule according to your availability
  • Minimum weekly or bi-weekly hourly commitment to fit your needs

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and assess previous and current medical history, medical conditions, and medication usage
  • Educate patients on their medical conditions and prescribed medications
  • Identify care gaps and recommend next steps for better health outcomes
  • No need to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Arkansas (AR)
  • Current CPR certification (BLS required)
  • Board certification through AANP or ANCC
  • Minimum six months of recent Nurse Practitioner experience (preferred)
  • MSN from an accredited nursing program

As part of this per diem position, youll receive compensation of $80$90 for each completed evaluation, along with a mileage stipend and paid administrative time.
