Deliver compassionate care and make a positive impact in the community with this per diem Nurse Practitioner opportunity specializing in home health risk assessments. This role offers the flexibility to create your own schedule with shifts available seven days a week, allowing you to maintain an ideal work-life balance. Assignments will be conducted throughout Pitt County, including Greenville, Winterville, and Ayden, with each assessment typically lasting about 45 minutes.

Schedule:

• Flexible shifts between Monday and Sunday
• Set your own schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct in-home annual health risk assessments for Medicare Advantage plan members.
• Evaluate comprehensive medical and medication histories, as well as current and past health conditions.
• Educate members on their medical conditions and medication regimens.
• Identify and address any care gaps by providing informed recommendations.
• No requirement to create treatment plans or prescribe medications.
• Serve a vital role in post-discharge visits and annual assessments, ensuring continuity of care for each member.

Qualifications:

• Active Nurse Practitioner license in North Carolina.
• Current CPR certification (BLS).
• Board Certification through AANP or ANCC.
• Minimum of 6 months recent NP experience preferred.
• MSN from an accredited nursing school.
