Job Description
Experienced Nurse Practitioners are needed to perform home health risk assessments for Medicare Advantage plan members in various areas within Pitt County. This per diem opportunity provides the autonomy to create your own schedule with flexible shifts available any day of the week, Monday through Sunday, and a minimum commitment on a weekly or bi-weekly basis. Assignments will take place in Greenville, Winterville, Ayden, and surrounding communities throughout Pitt County.
Schedule:
- Flexible shifts between Monday and Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate past and current medical history, chronic conditions, and medication regimens
- Educate patients on health conditions and proper medication usage
- Identify gaps in care and provide recommendations to improve health outcomes
- No treatment planning or prescribing of medications required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification (AANP or ANCC)
- MSN from an accredited nursing program
- Current CPR certification (BLS)
- At least 6 months of recent NP experience preferred
