Job Description

Deliver high-quality care to patients where they live as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you will play an important role in evaluating and educating Medicare Advantage plan members within Beaufort County, North Carolinaincluding towns such as Bath, Pantego, and Pinetown. Each home visit typically lasts around 45 minutes, encompassing a variety of needs, from annual health reviews to post-discharge check-ins. Assignments may vary by shift according to member locations.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling with the ability to select preferred days and times
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete comprehensive annual and post-discharge risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, including conditions and medications
  • Educate patients on their health conditions and prescribed medications
  • Identify gaps in care and make appropriate recommendations
  • Focus solely on assessmentsno treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Minimum of 6 months experience as a Nurse Practitioner is preferred
  • Master of Science in Nursing (MSN) from an accredited institution
