Job Description
Nurse Practitioners with a passion for delivering patient-centered care in home settings can find rewarding opportunities conducting risk assessments for Medicare Advantage plan members. These evaluations support preventive care and enhance overall health outcomes by assessing individuals in the comfort of their homes across Greenville, Winterville, Ayden, and other locations throughout Pitt County.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform home health risk assessments, typically lasting around 45 minutes per visit
- Review and evaluate individuals medical and medication histories
- Educate patients on their diagnoses and medications to promote understanding and management
- Identify potential gaps in patient care, offering appropriate recommendations to close those gaps
- Visits do not involve creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Master of Science in Nursing (MSN) from an accredited educational institution
- Current CPR certification (BLS)
- Certified by AANP or ANCC
- Minimum of 6 months of recent nurse practitioner experience preferred
This per diem assignment offers Nurse Practitioners an attractive opportunity with compensation of $80-$90 per evaluation, including a mileage stipend and compensation for administrative time.
