Are you an experienced Nurse Practitioner seeking a flexible per diem opportunity in home health? This role involves conducting home health risk assessments for Medicare Advantage plan members across Pitt County, including areas such as Greenville, Winterville, and Ayden. Assessments are typically 45 minutes and may cover annual health checkups or post-discharge evaluations. Each shift, you could visit different scheduled locations within the county.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and document prior/current medical history, conditions, and medications
- Educate patients on disease management and medication adherence
- Identify care gaps and recommend follow-up actions as needed
- No treatment planning or prescribing responsibilities required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Valid MSN degree from an accredited program
- At least 6 months of recent Nurse Practitioner experience preferred
Compensation: Earn $80$90 per evaluation completed, along with a mileage stipend and hourly pay for administrative tasks. This structure provides excellent earning potential and covers time spent in documentation and travel.
