Job Description
Make a meaningful impact in patient care by conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Grant County, Indiana. This per diem opportunity allows Nurse Practitioners to leverage their expertise in a flexible, patient-centric settingdelivering assessments in residents homes across Marion, Gas City, Upland, and surrounding areas.
Schedule/Hours:
- Flexible shifts available between Monday and Sunday
- Set your own schedule to fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients medical history, ongoing conditions, and medication use
- Educate patients regarding their medical status and prescribed medications
- Identify and address potential gaps in care, offering recommendations for improved outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana (IN)
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited school of nursing
- At least 6 months of recent experience as a Nurse Practitioner preferred
