Job Description
Nurse Practitioners with home health expertise are sought to perform comprehensive health risk assessments for Medicare Advantage plan members in Forsyth County. As a per diem provider, you will visit clients in their homesincluding those in communities such as Kernersville, Clemmons, and Lewisvilleconducting assessments that support proactive healthcare for seniors and recently discharged patients.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Carry out annual and post-discharge home health risk assessments, each averaging 45 minutes
- Review and document past and current medical histories, existing conditions, and medications
- Offer patient education on health conditions, medications, and preventive care
- Identify gaps in care and make recommendations for ongoing follow-up
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least six months of recent Nurse Practitioner experience preferred
