Professionals in this per diem role play an essential part in providing high-quality home health risk assessments for Medicare Advantage plan members throughout Guilford County. Assessments support members residing across diverse communities, including Greensboro and nearby areas such as Summerfield, Oak Ridge, and Stokesdale. With the flexibility to create your own schedule, this opportunity is ideal for Nurse Practitioners seeking control over their work-life balance.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling options
• Minimum weekly or bi-weekly hourly commitment required

Primary Responsibilities:

• Conduct comprehensive home health risk assessments (approximately 45 minutes per visit)
• Review and evaluate each members prior/current medical history, medical conditions, and medication history
• Educate members about their health conditions and prescribed medications
• Identify gaps in care and suggest actionable recommendations
• Perform only assessmentsno need to develop treatment plans or prescribe medications

Desired Qualifications:

• Active Nurse Practitioner license (NC)
• Board Certification via AANP or ANCC
• Completion of MSN degree from an accredited nursing program
• Current CPR certification (BLS)
• At least 6 months of recent NP experience is preferred

You will receive $80-$90 per evaluation, along with additional compensation for mileage and administrative time, reflecting the importance and respect given to this vital role.
