Take part in an impactful home health initiative providing risk assessments to Medicare Advantage members throughout Fayette County. In this per diem opportunity, you will work autonomously, conducting comprehensive home health visits and evaluations on a flexible schedule that aligns with your lifestyle.
Schedule:
- Flexible shifts available seven days a week
- Self-schedulingchoose your preferred days and times
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete annual and post-discharge home health risk assessments with Medicare Advantage members in their homes
- Assess prior and current medical history, medical conditions, and medication usage
- Offer thorough education on disease states, conditions, and medications
- Identify and communicate gaps in care, providing recommendations for continued management
- Focus on evaluationno requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification (AANP or ANCC)
- Current BLS (CPR) certification
- Master of Science in Nursing (MSN) degree from an accredited institution
- At least six months of recent Nurse Practitioner experience preferred
This role provides competitive pay of $80$90 per evaluation, plus a mileage stipend and compensation for administrative time, recognizing your expertise and effort.
