Job Description

Schedule:

  • Highly flexible shifts available Monday through Sunday

Role Overview:

  • Conduct comprehensive annual or post-discharge health risk assessments in patients homes
  • Average assessment duration is approximately 45 minutes per home
  • Gather detailed data on past and current medical history, chronic conditions, and current medications
  • Deliver education to members and their families regarding their health conditions and appropriate medication use
  • Identify any gaps in existing care and provide actionable recommendations for resolution
  • No prescribing or treatment plan creation is required in this assessment-focused position

Qualifications:

  • Active Nurse Practitioner license for Ohio
  • Board Certification as a Nurse Practitioner (AANP or ANCC)
  • Current CPR certification (BLS required)
  • MSN degree from an accredited nursing program
  • A minimum of 6 months of experience working as a Nurse Practitioner is preferred
