Expand your clinical experience with a per diem opportunity focusing on home health risk assessments for Medicare Advantage plan members throughout Highland County, OHincluding Greenfield, Hillsboro, and Leesburg. Schedule:
- Highly flexible shifts available Monday through Sunday

- Commit to either weekly or bi-weekly minimum hours as it fits your availability
Role Overview:
- Conduct comprehensive annual or post-discharge health risk assessments in patients homes
- Average assessment duration is approximately 45 minutes per home
- Gather detailed data on past and current medical history, chronic conditions, and current medications
- Deliver education to members and their families regarding their health conditions and appropriate medication use
- Identify any gaps in existing care and provide actionable recommendations for resolution
- No prescribing or treatment plan creation is required in this assessment-focused position
Qualifications:
- Active Nurse Practitioner license for Ohio
- Board Certification as a Nurse Practitioner (AANP or ANCC)
- Current CPR certification (BLS required)
- MSN degree from an accredited nursing program
- A minimum of 6 months of experience working as a Nurse Practitioner is preferred
