Job Description

A rewarding opportunity is available for a Nurse Practitioner experienced in home health assessments. This per diem role involves conducting risk assessments for Medicare Advantage plan members throughout Shelby County, including locations such as Shelbyville, Fairland, and Morristown. The position is designed for those seeking flexibility, autonomy, and the chance to make a direct impact on patient health outcomes.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Set your own schedule based on availability
  • Choose minimum weekly or bi-weekly hourly commitments

Responsibilities include:

  • Completing comprehensive home health risk assessments for Medicare Advantage members in their residences
  • Reviewing and documenting prior and current medical histories, existing conditions, and medication regimens
  • Providing education regarding health conditions and medication management to patients
  • Identifying gaps in health care and offering recommendations to close those gaps
  • No requirement to create treatment plans or prescribe medications during visits

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • At least 6 months of recent NP experience is preferred
  • Master of Science in Nursing (MSN) from an accredited institution
