Job Description

Embark on a rewarding per diem opportunity delivering essential healthcare services directly in patients homes. This position focuses on conducting home health risk assessments with Medicare Advantage members throughout Hancock County, including areas such as Greenfield, McCordsville, and Fortville. This setting provides autonomy and meaningful patient interactions while allowing you to tailor your schedule to fit your lifestyle.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members
  • Evaluate patients medical and medication histories
  • Assess current and prior medical conditions
  • Identify care gaps and recommend appropriate follow-ups
  • Educate patients on health conditions and medication management
  • No requirement to develop treatment plans or prescribe medication

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN from an accredited school of nursing
  • At least six months of recent experience as an NP is preferred

This per diem opportunity offers competitive compensation of $80-$90 per evaluation, in addition to mileage reimbursement and compensation for administrative time.
