Job Description

Advance your clinical practice through meaningful work conducting in-home health risk assessments for Medicare Advantage plan members. This per diem role offers a highly flexible schedule and the chance to make a measurable impact on patients' health across Hamilton County, IN, including Carmel, Fishers, and Noblesville. Set your own shifts between Monday and Sunday to suit your lifestyle, with the ability to commit weekly or bi-weekly minimum hours as desired.

Primary Responsibilities:

  • Conduct comprehensive home health risk assessments in a patients home, typically lasting 45 minutes per visit
  • Evaluate medical and medication histories, along with current and prior medical conditions
  • Educate patients on health conditions and medication use
  • Identify gaps in care and offer recommendations to enhance patient outcomes
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in IN
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited school of nursing
  • At least 6 months of recent experience as a Nurse Practitioner preferred

Expect to earn $80$90 per evaluation, complemented by a mileage stipend and administrative time, while focusing on clinical assessment and patient educationwithout the demands of ongoing patient management or on-call requirements.
