Advance your career as a Nurse Practitioner specializing in home health, providing essential care and support to Medicare Advantage plan members throughout Clay County, INincluding Brazil, Clay City, and Knightsville. This per diem opportunity offers a flexible, self-scheduling model, allowing you to tailor your shifts to fit your availability, with minimum weekly or bi-weekly hourly commitments.

- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hour requirements
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members during home visits
- Review and evaluate prior and current medical history, ongoing health conditions, and medication regimens
- Provide targeted education regarding chronic conditions and medication usage to patients and their families
- Identify care gaps and recommend actions to optimize patient health outcomes
- Visits may include annual health assessments or post-discharge evaluations; there is no requirement for prescribing medication or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of 6 months of current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited nursing program
This home health role is compensated at $80$90 per evaluation, plus a mileage stipend and additional administrative time, ensuring your expertise and commitment are recognized and rewarded.
