This per diem opportunity provides the autonomy to design your schedule and make a difference for individuals across Clay County, Indiana. You'll perform comprehensive home health risk assessments, serving Medicare Advantage plan members in communities such as Brazil, Clay City, and Knightsville.
Schedule:
- Flexible shifts available between Monday and Sunday
- Opportunity to set your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and evaluate patient medical histories, current medical conditions, and medications
- Educate patients on their conditions and medication regimens
- Identify gaps in care and offer recommendations to enhance patient outcomes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- Minimum of 6 months' experience as a Nurse Practitioner preferred
Compensation for this role includes $80$90 per evaluation, along with a mileage stipend and remuneration for administrative time.
