Schedule:
- Flexible shifts available seven days a week
- Design your own work schedule
- Choose a minimum hourly commitment weekly or bi-weekly
Responsibilities:
- Perform comprehensive home health risk assessments, averaging 45 minutes per patient, for individuals residing in areas including Harrison, Diamond City, and Bergman
- Review and evaluate prior and current medical history, active medical conditions, and medication profiles
- Educate patients regarding diagnoses and prescribed medications to encourage optimal health outcomes
- Detect gaps in care and make actionable recommendations to support patient well-being
- Visits may include annual health evaluations or focused post-discharge assessments
- No treatment plan creation or medication prescribing required
Qualifications:
- Active and unrestricted Nurse Practitioner license in Arkansas
- Current CPR (BLS) certification
- Certification with AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months recent experience as an NP preferred
