Shape the future of patient-centered care by providing vital in-home health assessments across Carroll County, AR. As a per diem Nurse Practitioner specializing in home health, youll have the flexibility to create your own schedule and work within Berryville, Green Forest, Holiday Island, and surrounding areas. In this meaningful position, youll conduct Medicare Advantage risk assessmentseach taking approximately 45 minutesdelivering crucial insights without the need to prescribe medications or develop treatment plans.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: select shifts that fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, ongoing conditions, and all medications
• Educate members on their health conditions and prescribed medications
• Identify care gaps, making clear recommendations for next steps
• Collect clinical data and provide clear, concise documentation of each assessment

Qualifications:

• Active Nurse Practitioner license in Arkansas
• Board Certification through AANP or ANCC required
• MSN degree from an accredited program
• Valid CPR (BLS) certification
• At least six months of experience as a Nurse Practitioner is preferred

For each completed evaluation, you will receive $80-$90, plus a mileage stipend and compensation for administrative time, recognizing your professionalism and the value of your expertise.
