Step into a vital role providing home health risk assessments to Medicare Advantage members in Carroll County, AR. This per diem opportunity offers flexibility, autonomy, and the chance to make a meaningful impact within the community, including Berryville, Green Forest, and Holiday Island.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit personal and professional needs
- Minimum weekly or bi-weekly hourly commitment tailored to your availability
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Conduct thorough evaluations of patients prior and current medical history, medical conditions, and medications
- Offer clear education regarding diagnoses and medication usage
- Identify care gaps and provide tailored recommendations to help resolve them
- No requirements for treatment plan creation or medication prescribing
Qualifications:
- Valid Nurse Practitioner license in Arkansas
- Current CPR (BLS) certification
- Board certification from AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing from an accredited program
Assessments typically take about 45 minutes per visit, offering the ability to effectively manage your workload and maximize your impact. You will receive $80$90 for each evaluation, along with a stipend for mileage and administrative time.
