Job Description
Deliver high-quality patient care and independent clinical assessments as a Nurse Practitioner specializing in home health. In this per diem opportunity, you'll perform comprehensive home health risk assessments for Medicare Advantage members throughout Weld County, including visits to Greeley, Longmont, Windsor, and nearby areas. Each assessment lasts approximately 45 minutes and may involve annual health evaluations or post-discharge follow-ups.
Schedule:

- Minimum weekly or bi-weekly hourly commitment to fit your lifestyle
Responsibilities:
- Complete in-home risk assessments on Medicare Advantage plan members
- Review and document medical history, existing conditions, and medication use
- Communicate important information and education regarding medical conditions and prescriptions
- Identify potential gaps in care, recommending steps for resolution
- No requirement to establish treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Colorado
- Current BLS/CPR certification
- Board Certification (AANP or ANCC)
- Minimum of 6 months recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This unique role offers compensation of $80-$90 per evaluation, with additional mileage stipend and paid administrative time.
