Deliver high-quality care and support by conducting home health risk assessments in a per diem capacity for patients in Lancaster County, SC, including communities such as Lancaster, Indian Land, and Kershaw. As a Nurse Practitioner, you will provide essential evaluations and health education for Medicare Advantage plan members, ensuring their care needs are thoroughly assessed and addressed.

Schedule:

• Flexible shifts available Monday through Sunday
• Autonomy to create your own schedule
• Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

• Complete annual health risk assessments and post-discharge visits in home settings
• Review and evaluate patients current and historical medical information
• Assess medication usage and provide educational guidance on conditions and treatments
• Identify potential gaps in care and recommend solutions to promote better health outcomes
• Focus on assessments only; treatment planning and prescription management are not required

Qualifications:

• Active Nurse Practitioner license in South Carolina
• Board Certified by AANP or ANCC
• Current CPR (BLS) certification
• Completion of an MSN degree from an accredited nursing program
• At least 6 months of current Nurse Practitioner experience preferred

For each evaluation, you will be compensated at a rate of $80-$90, along with an additional mileage stipend and support for administrative time.
