Advance your healthcare career with a per diem opportunity focused on home health risk assessments for Medicare Advantage members. In this role, you will visit patients in their homes throughout Baldwin County for evaluations that typically last around 45 minutes. Visits will include annual health assessments and post-discharge check-ins to support better patient outcomes.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday

- Minimal weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete home-based risk assessments for Medicare Advantage health plan members
- Review and document patients medical history, existing conditions, and medication usage
- Offer patient education related to health conditions and prescription regimens
- Identify gaps in care and recommend actionable solutions
- No treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in the state of Georgia
- Current Basic Life Support (BLS) certification
- Board certification from AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience is preferred
- Master of Science in Nursing (MSN) from an accredited institution
