Take on a rewarding role conducting home health risk assessments for Medicare Advantage members throughout Burke County, NC. As a Nurse Practitioner specializing in home health, you'll provide essential care to individuals in their homes, ensuring their health needs are carefully evaluated and addressed. This per diem opportunity offers flexibility in your schedule while allowing you to provide impactful, patient-centered support in our community.
Schedule:
- Flexible shifts available Monday through Sunday
- Opportunity to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical history, medical conditions, and medication regimens
- Educate patients on their health conditions and prescribed medications
- Identify any gaps in care and recommend actionable solutions
- Visits may include annual assessments, post-discharge visits, and varied patient needs
- No treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Board Certification via AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited school of nursing
- At least 6 months of experience as a Nurse Practitioner is preferred
This position provides competitive compensation of $80-$90 per evaluation, with additional stipends for mileage and administrative time.
