Bring your advanced clinical expertise to a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. You will serve Medicare Advantage plan members throughout St. Clair County, Illinois, including areas such as Belleville, O'Fallon, and East St. Louis. In this role, youll have the autonomy to manage your own calendar while contributing to improved healthcare outcomes for members in their homes.

Schedule:

• Flexible shifts available seven days a week
• Choose your own schedule
• Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

• Complete annual and post-discharge home health risk assessments (approximately 45 minutes each)
• Assess patient medical histories, current conditions, and medication profiles
• Educate members about their medical conditions and prescribed medications
• Recognize and document any gaps in care, providing recommendations for follow-up
• No treatment planning or prescribing responsibilities required

Desired Qualifications:

• Active Nurse Practitioner license in Illinois
• Master of Science in Nursing (MSN) from an accredited institution
• Board Certification through AANP or ANCC
• Current, valid CPR certification (BLS)
• At least six months of recent Nurse Practitioner experience preferred
