Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Meet a minimum hourly commitment weekly or bi-weekly

Core Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage patients in their homes
• Assess current and prior medical history, active medical conditions, and medication regimens
• Educate members on their health conditions and proper medication use
• Identify gaps in care and provide appropriate recommendations for care improvement
• Visits are assessment-onlyno treatment planning or medication prescribing involved
• Each evaluation typically takes about 45 minutes

Qualifications Sought:

• Active Nurse Practitioner license in Illinois
• Current Basic Life Support (BLS/CPR) certification
• Board Certification as a Nurse Practitioner through AANP or ANCC
• MSN degree from an accredited nursing program
• Minimum of 6 months recent NP experience preferred

Compensation is provided at $80-$90 per completed evaluation, with an additional mileage stipend and pay for administrative time.
