Step into a dynamic per diem opportunity as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage members throughout St. Clair County, Illinois, including Belleville, O'Fallon, and East St. Louis. This flexible position empowers you to create your own schedule and deliver high-quality, patient-centered care in the comfort of patients' homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduled shifts for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive annual home health risk assessments (approximately 45 minutes per visit)
- Evaluate patient medical history, existing conditions, and medication usage
- Deliver effective education regarding medical conditions and medications to members
- Identify care gaps and collaborate to recommend solutions, enhancing patient outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Illinois
- Board certification via AANP or ANCC
- MSN degree from an accredited nursing school
- Current CPR certification (BLS)
- At least 6 months current Nurse Practitioner experience preferred
