Job Description
Serve a vital role in patient care by conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout St. Clair County, Illinois. This per diem Nurse Practitioner opportunity offers the flexibility to create your own schedule and engage in meaningful interactions with patients in their homes, including areas such as Belleville, OFallon, and East St. Louis.
Schedule
- Flexible shifts available any day of the week, Monday through Sunday
- Design your own work schedule
- Minimum hourly commitment each week or every two weeks
Key Responsibilities
- Conduct annual health and post-discharge home health risk assessments (approximately 45 minutes per visit)
- Evaluate patients past and present medical histories, conditions, and medication regimens
- Educate members on their medical conditions and medications
- Identify gaps in care and suggest strategies or resources to address these
- No responsibility for creating treatment plans or prescribing medications
Qualifications
- Active Nurse Practitioner license in Illinois
- Valid CPR (BLS) certification
- Board Certified through AANP or ANCC
- MSN degree from an accredited nursing program
- At least six months of current NP experience preferred
