Offering per diem flexibility, this opportunity is ideal for experienced Nurse Practitioners skilled in home health clinical assessments. As part of a team delivering care in St. Clair Countyincluding areas such as Belleville, O'Fallon, and East St. Louisyoull conduct focused risk assessments for Medicare Advantage plan members. Each home health risk assessment visit typically lasts about 45 minutes and may include annual health assessments or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule; work-life balance prioritized
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review prior and current medical history, active conditions, and medication lists
- Provide education to clients regarding health conditions and medications
- Identify care gaps and offer recommendations for improvement
- No prescribing of medications or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in Illinois
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum MSN degree from an accredited nursing program
- At least 6 months of current NP experience preferred
