Job Description

Experienced Nurse Practitioners are needed for a per diem home health role providing risk assessments to Medicare Advantage plan members throughout Madison County, Illinois. In this flexible position, you will conduct in-home health evaluations that may include annual assessments or post-discharge visits, spending approximately 45 minutes per appointment. Assessments are scheduled in a range of locations within Madison County, including Granite City, Alton, and Edwardsville, with assignments varying by shift.

Schedule:

  • Flexible shifts available any day: Monday through Sunday
  • Customize your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive annual health risk assessments in patients homes
  • Review and evaluate medical history, current conditions, and medication lists
  • Educate members on health conditions and medication usage
  • Identify care gaps and recommend steps to address those gaps
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Board certification (AANP or ANCC)
  • MSN from an accredited nursing school
  • Current BLS (CPR) certification
  • Minimum of 6 months' NP experience preferred
