Job Description

Advance your nursing practice in a dynamic per diem opportunity as a Nurse Practitioner supporting home health risk assessments within Idaho County. This role offers autonomy, varied patient interactions, and the flexibility to create your own work schedule. You will travel within locations such as Grangeville, Cottonwood, and Kamiah, delivering high-quality care to Medicare Advantage plan members in their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling enables completion of a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform in-home annual health risk assessments for Medicare Advantage members
  • Evaluate patients prior and current medical conditions, medical history, and medication usage
  • Offer education regarding medical conditions and medication regimens
  • Identify care gaps and recommend steps to address them
  • Visits are focused on assessment onlyno treatment planning or prescription writing required
  • Each evaluation typically takes about 45 minutes

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Master of Science in Nursing (MSN) from an accredited institution
  • Board Certified through AANP or ANCC
  • Current CPR certification (BLS)
  • At least 6 months of recent nurse practitioner clinical experience preferred
