Job Description

Deliver high-quality healthcare in a diverse home health setting as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers meaningful patient interaction and professional autonomy, serving Medicare Advantage members across Idaho County, including Grangeville, Cottonwood, and Kamiah.

Schedule:

  • Flexible shifts available between Monday and Sunday

  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct in-home annual risk assessments for Medicare Advantage members
  • Review and evaluate patients prior and current medical history, conditions, and medication usage
  • Provide patient education regarding medical conditions and prescribed medications
  • Identify gaps in care and recommend next steps for ongoing healthcare needs
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current CPR certification (BLS)
  • Board certification via AANP or ANCC
  • Six months or more of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
