This per diem opportunity is ideal for a Nurse Practitioner seeking flexibility and autonomy in a home health setting. You will conduct comprehensive home health risk assessments for Medicare Advantage plan members throughout Dare County, NCincluding areas like Kill Devil Hills, Atlantic township, Nags Head township, and Hatteras township. Each visit averages 45 minutes and may include annual health assessments or post-discharge evaluations. Scheduling offers substantial flexibility, with shifts available any day of the week and the ability to create your own working calendar.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual risk assessments for Medicare Advantage members in their homes
- Review and evaluate patients prior and current medical histories, existing conditions, and medication profiles
- Deliver patient education related to medical conditions and treatment adherence
- Identify gaps in care and provide actionable recommendations to enhance patient outcomes
- No treatment planning or medication prescribing is needed
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
This position offers compensation of $80$90 per evaluation, along with a mileage stipend and additional pay for administrative time.
